Origins Chiropractic and Wellness is a warm and inviting place that doesn’t feel like your typical medical office. We ask that our new patients print off the new patient paperwork below and bring it with you to your first appointment. If you are unable to do so, arrive about 15 minutes prior to your scheduled appointment time to fill it out.

Your First Visit
When you arrive, you’ll sit down with one of our doctors to go over your medical history and talk about your concerns. The next step will be the examination, which involves:
- Chiropractic examination
- Range of motion and posture evaluation
- Initial X-rays
After the examination you’ll receive your first chiropractic adjustment. Our adjustments are very specific and based off measurements taken from your x-rays as well as other objective findings that were observed during examination. Included in the first visit fee is a post upper cervical x-ray. This allows us to instantly see how the alignment has changed and ensures pressure has been removed from the brainstem, freeing CSF, oxygen and blood flow to and from the brain. The initial visit is the most comprehensive and takes approximately one hour.
Following Up
You will be sent home with instructions for aftercare and what to expect following your first adjustment. We perform our chiropractic examination and posture evaluation at every subsequent visit to determine if you are holding your adjustment, and to determine if you need an adjustment that day. We want to get you to the point where spinal check-ups are periodic, so you are spending more time out doing the things you love than in our office. Subsequent visits take approximately 15 minutes.
Payment Information
Payment is due at the time of service. We accept cash, personal checks, HSA, Care Credit and all major credit cards. We do not directly bill insurance, however, we can provide you with an itemized receipt with insurance codes that you may submit to your insurance. We do this to stay true to our philosophy of providing 100% patient-focused care that is necessary, appropriate, and personalized. This model of care ends up being more cost-effective as our goal is to get your body to hold adjustments longer so visits become less frequent.
